ORDERING INSTRUCTIONS:
Either print the Order Form or fill it out online indicating quantity, editor, title, and price of publications desired and the total cost of volume(s). Enter the order subtotal in the space provided at the bottom of the form. Add the appropriate postage and handling charge. Orders within Illinois must also add 6.25% sales tax in addition to the postage/handling fee. For orders of more than eight books, for fast delivery, and and for foreign shipping rates, contact Kathy Lundeen at tktdc@siu.edu. Provide name and the address where the order should be mailed. Send order form with payment due to the address below. Payment is due in U.S. currency, U.S. bank draft, or international money order.
